Your First Visit
Thank you for selecting our office to serve your dental needs. Our entire team is dedicated to providing the highest quality dental care and service to our patients. We take great pride in each staff member's training.
So that we may all enjoy a smooth working relationship, we ask that you take a few minutes to fill out the New Patient Medical / Dental History form prior to your first appointment. This form is in Adobe Portable Download Format (PDF) and can be found below and printed on your computer. After filling out this form, you can send it to our office or bring it with you to your first appointment. This will expedite your wait time and allow us to spend more time with you.
If you will be using dental insurance to help you with your fees, please have your insurance information available at your appointment so we can properly send your claim forms. If you have any questions, please do not hesitate to call us at (440) 646-0477.

If you have dental insurance, our goal is to maximize your insurance benefits and make any remaining balance easily affordable. Insurance or not we will help you get the most from your budget so you can benefit from the quality of care you need or desire. We accept the following payment options:
Cash, Visa, MasterCard, Money Order, Personal Checks and CareCredit.

| Our practice accepts most insurance plans and as a courtesy to our patients, we will submit your dental insurance claims on your behalf. You will be expected to pay an estimated co-payment at the time of treatment. We ask that you keep us updated with your current insurance information so we may accurately bill your insurance in a timely manner. |
| Also, for your convenience, we are pleased to accept CareCredit monthy payment plans, the flexible, affordable way to pay for healthcare. Just give us a call so we can help you - it's easy. |
